Guidelines for Verification of Student Residency
Upon presentation of a new student at school, the parent/guardian must present the following forms of documentation along with the child’s original birth certificate:
-Three (3) proofs of residency such as a utility bill, deed, lease, or tax bill.
-Court orders, State agency agreements and other evidence of court or agency placement or directives.
-A residency check by the district attendance officer may also be required.
The voluntary program is in session from September through June and is an integral part of the Linden Public Schools. Teachers certified in Early Childhood Education will provide your child with a rewarding and enjoyable educational experience five days per week. There will be a limited number of full-day spaces available. A responsible adult must accompany your child to and from school.
Registration In-person registration will be held at Schools 2, 4, 5, 6, 8, 9 and 10 on:
March 6, 2012 – 9 am-12 pm
March 8, 2012 – 9 am-12 noon; 5:30 pm-7:30 pm
March 14, 2012 – 9 am-12 noon; 5:30 pm-7:30 pm
March 16, 2012 – 12:30 pm-2:30 pm
Half-day 3 year old classes
-Child must be 3 years old on or before October 1, 2012.
Full-day 4 year old classes
-Child must be 4 years old on or before October 1, 2012. -Linden residents only may be enrolled in the program.
-Child must register and attend the school in the area in which he/she lives.
-Child must be toilet trained to be eligible.
Provide Verification of Immunizations:
-D.P.T. – 4 doses
-POLIO – 3 doses
-MEASLES, MUMPS, RUBELLA (MMR): 1 dose, live (must be given after first birthday)
-H.I.B., VARICELLA and PNEUMOCOCCAL- 1 dose of each (must be given after first birthday)
-HEPATITIS B – 3 doses
-INFLUENZA – one dose between Sept. 1 and Dec. 31 of entry year
-MEDICAL EXAMINATION – must be performed no more than 365 days prior to entry into school and be documented on the approved form.
