New Registration Requirements:

•  1. Proof of Residency
•  2. Health Records
•  3. Birth Certificate & Social Security
•  4. Transfer Card from previous school
•  5. Report Card for this year and previous high school grades (transcript)
•  6. School profile
•  7. State or standardized test results (GEPA, HESPA, TERRA NOVA)
•  8. Guardianship Papers (if needed)
•  9. Eighth Grade diploma
• 10. IEP (for Special Education)

Registrant Must Complete the Following Forms:

• 1. Parent Consent Form for the records
• 2. Blue Emergency Card
• 3. Medicaid Form (if special education)
• 4. NJ Kids Care Health Application (all new registrants)
• 5. Lunch Forms (all new registrants)
• 6. Transportation Form (if needed)

** Approximate time required - 1 hour to complete forms
